In one longitudinal study of depression's natural course, 70 percent of subjects had reached clinical remission within six months. By fifteen months, only 6 percent had not yet remitted. The median time to recovery, depending on which of four remission-duration definitions researchers applied — needing two, four, six, or twelve consecutive symptom-free months to count as recovered — ranged from 3.0 to 12.0 months. That is the first fact this closing chapter has to hold onto, and it needs to be held carefully, because it is true and it is not the whole picture.
A separate three-year follow-up of hospitalized depressed patients — a more severely affected population than the community samples behind the numbers above — found a rougher course: 36 percent maintained remission across the full follow-up, 12 percent never reached remission at all, and 52 percent had a fluctuating course, moving in and out of symptoms over the three years. Both sets of numbers are correct. They describe different populations. A mild depressive episode treated in an outpatient setting and a severe episode requiring hospitalization are not the same statistical event, and stating one figure as if it applied equally to both would misrepresent the evidence in the direction of false comfort. The honest sentence is: depression's default course, especially outside hospital settings, is episodic, with most people eventually remitting — but severity and setting change the odds substantially, and no single percentage covers everyone carrying the diagnosis.
Remission, when it happens, is also not always permanent, and the data on what comes after deserve equal weight. In one cohort, 40 percent of patients who reached remission relapsed within fifteen months, and every one of those relapses occurred within the first ten months — meaning the risk clusters early rather than spreading evenly across time. Other literature reports relapse rates as high as 50 percent within two years of remission. Recurrence risk, moreover, rises with each additional past episode: a first episode carries lower odds of recurrence than a third or fourth. Depression's episodic pattern is not "cured once, resolved forever" for a meaningful share of people who experience it. It is closer to a condition that comes, often goes, and for some returns — with the timing and odds of that return depending on history, not fixed in advance.
What treatment actually delivers, measured honestly
The largest real-world effectiveness trial of depression treatment ever conducted, STAR*D, funded by the National Institute of Mental Health and originally published by Trivedi and colleagues in 2006, offers the clearest single number for what a first medication trial actually does. In Step 1, patients starting on citalopram, remission was reported at 36.8 percent — treatment-naive patients (those in their first depressive episode with no prior treatment) remitted at 43 percent, patients with a treatment history at 36 percent.
That original figure has itself been reexamined. A 2023 to 2025 reanalysis published in BMJ, using a Restoring Invisible and Abandoned Trials (RIAT) approach — a method for reconstructing a trial's results strictly from its original, pre-registered protocol criteria, correcting for any drift in how outcomes were defined after the fact — found a stricter first-step remission rate of 35.0 percent, close to but measurably below the originally reported number. This is not a minor footnote. It is a documented case of one of the most influential trials in psychiatric history having its own headline statistics disputed and partially revised downward by independent reanalysis using the trial's own preregistered rules. That correction belongs in the record for the same reason the oxytocin replication and the candidate-gene collapse belong in this book: a number that is repeated often enough starts to sound more solid than the evidence underneath it, and the discipline of going back to the original protocol is what keeps the number honest.
Read plainly, roughly one in three patients remits on the first medication tried. That is high enough to be a real, clinically meaningful effect — not a placebo-sized illusion. It is also low enough that most people need a second step, a different medication, or a different approach entirely. STAR*D was never a study that found "the" treatment for depression. It was a study that found, empirically and at scale, that no single first-line treatment works for most people — which is itself evidence for everything chapter eleven established about biological heterogeneity. A disorder with one cause would be expected to have one treatment with a high response rate. This one does not.
What is established, and what is not
It is worth naming, plainly, which claims in this book rest on settled ground and which remain live hypotheses, because the two have been sitting side by side across the last several chapters and the difference between them is the entire point.
Established, in the sense of replicated across independent large samples: depression is heritable — twin studies put the figure near 37 percent — but that heritability is spread across many genetic variants of individually tiny effect, not concentrated in a small number of causal genes; the eighteen most historically studied candidate genes showed no reliable direct or gene-by-environment effect once tested in a sample of 621,214 people with more than 99.99 percent power to detect the originally claimed effect sizes. Established: social isolation, loneliness, and living alone each independently raise all-cause mortality risk, at magnitudes — 29 percent, 26 percent, and 32 percent respectively — that held consistently across gender, geography, and follow-up length in a pooled sample exceeding 3.4 million people. Established: depression correlates with measurable but non-universal inflammatory signatures, present in roughly a quarter to just over half of depressed patients depending on the threshold used, not in all of them. Established: for most people, especially outside hospital settings, a depressive episode is time-limited, with remission typically arriving within months rather than persisting indefinitely — while relapse remains a real and clustered risk for a meaningful fraction of those who do remit.
Still hypothesis, explicitly flagged as such in the literature that produced it: that vagal afferent signaling — the eighty percent of the vagus nerve's fibers that carry information from body to brain — plays a causal role in human mood disorders, rather than merely correlating with them; that inflammation is a cause of depression in the patients who show it, rather than a consequence of it or an incidental marker of a distinct biological subtype; that oxytocin functions as a meaningful driver of trust and bonding in humans at the doses and delivery methods tested, a claim two independent, adequately powered replications failed to support; and that the neuroimaging subtypes now being identified within the single diagnostic category of major depressive disorder can be reliably separated into stable categories a clinician could use — a project the literature itself currently calls "largely unclear."
The honest position, then, is neither of the two stories that circulate most widely. It is not "depression is a chemical imbalance with a known biological cause and a reliable fix" — the candidate-gene collapse and the absence of any validated biomarker rule that story out directly. It is also not "nothing is known and the whole field is guessing" — the isolation-mortality link, the polygenic genetic architecture, the STAR*D remission rates, and the episodic-course data are all real, replicated, quantified findings, not speculation. What is available is population-level regularity without individual-level prediction. Science can currently say, with real numbers behind it, what happens on average to large groups of people who share a diagnosis. It cannot yet say, for one specific person on one specific day, which subtype they carry, what their remission timeline will be, or which treatment will work on the first try.
That gap — between what is true of the group and what can be said of the one — is not a temporary embarrassment waiting on the next study to close it. It may be a structural feature of a disorder that a decade of imaging and genetics research has shown to be biologically plural rather than singular. A single label, "major depressive disorder," sits over several distinct underlying conditions that current science cannot yet tell apart in any one patient. That is the most precise sentence this book can offer about why prediction remains hard while the population statistics remain solid.
What the evidence does license, without overreaching past it, is this: the measured, replicated default course of a depressive episode, across the community samples where most episodes occur, is that it ends. Not always, not on a guaranteed schedule, not without real risk of return — the relapse data rule out any tidier version of that sentence. But 70 percent remitted within six months in one of the largest longitudinal analyses of this question, and that number was measured, not promised. It describes what happened to a large group of people who once carried the same diagnosis. It says nothing about any one person's calendar. Those two facts — the group statistic and its silence about the individual — are both true at once, and the evidence does not offer a way to collapse them into one.
Overturned if: a subsequent large-scale, adequately powered longitudinal cohort study found that the majority of community-treated major depressive episodes do not remit within the timeframes reported here — reversing the current population-level finding that most depressive episodes are time-limited.
